A 40 year old man is diagnosed as having a deep vein thrombosis. You start him on low molecular weight heparin and then warfarin. Four days later the international normalised ratio (INR) is 2.3. What should you do?

a)- Stop the heparin and continue the warfarin
b)-Continue the heparin for at least one more day
c)-stop the warfarin
