A man with penetrating chest trauma on left has now developed SOB his breathing has become labored no x-ray‚ he has got tachypnea and tachycardia. And there is also subcutaneous emphysema in neck ,on physical exam there is dull breath sounds plus mediastinum shift towards left side, what’s the diagnosis?

a- tension pneumothorax
b- hemopneumothorax
c- aortic dissection
d- cardiac temponade
e- aortic rupture
